Job Description:
Description Atlantic Health System is seeking a Per Diem Social
Worker for Case Management at Chilton Medical Center. Essential
Function/Responsibilities: Provide Social Work coverage to
facilitate discharges, meet with patients and families to assess
and provide discharge planning support, resources, referrals,
transfers to Extended Care and inpatient psychiatric facilities.
Work closely with the Healthcare Team to implement the plan of
care, obtain insurance authorizations as required, communicate with
community resources and area facilities. Phone, Fax and Electronic
documentation a must. Qualifications Minimum Education: MSW and LSW
Minimum Experience: 1-year medical setting (acute care hospital) or
1-year externship would be preferred
