The Housing Specialist provides supportive services including outreach, monitoring, assessment and service planning, support network building, consultation to families, side-by-side skill training and instruction in the use of community resources, including referrals to community assistance as needed. JOB DUTIES AND RESPONSIBILITIES:
Provide service coordination for an assigned group of clients.
Coordinate with BCDAC for all necessary information regarding housing grant requirements, including completing WITS/GPRA assessments as needed for every client.
Assist clients to find and maintain a safe and affordable place to live - apartment hunting, finding a roommate, landlord negotiation, cleaning, furnishing and decorating, and procuring necessities.
Ensure clients have adequate financial support (e.g., gain employment or apply for entitlements).
Teach money management skills and assist clients in accessing financial services.
Assist clients with accessing reliable transportation.
Participate in client-centered comprehensive assessment of psychiatric history, mental status, physical health, use of drugs or alcohol, education and employment, social development and functioning, activities of daily living, and family structure and relationships.
Complete referrals and follow-up to all behavioral supports identified through assessment process (i.e. a person is COD complete referral to recovery coaching, a person requires intensive support with vocational rehab.; complete referral to Reach and ensure connection). - If a person served is non-COD, work with COE team to address needs (i.e. CRS may be identified to work with individual wanting to go back to school).
Provide on-call crisis interventions on rotating schedule, flexibility with working some evenings as necessary.
Must have ability to sit for 2 total hours per day, and up to 1 total consecutive hour. Must be able to stand for 8 total hours per day and up to 2 total consecutive hours. Must be able to walk 6 total hours per day and up to 10 total consecutive minutes. Must have the ability to frequently use fingers/hands for fingering and handling. Must have the ability to use upper extremities for frequent pushing of objects up to 10 pounds and occasional pushing of objects up to 75 lbs. Must have the ability to transport patients weighing up to 250 lbs. via wheelchair, stretcher and/or bed. Must have the ability for frequent stooping and bending. Must have the ability for frequent reaching above shoulder level. Must have the ability for touching as it relates to feeling objects by touching with the skin, particularly that of fingertips. Must have the ability for hearing as it relates to normal conversation, high frequency and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision.
Bachelor's degree required.
PA Child Abuse/Criminal Clearances and FBI Background check required.
Displays sound theoretical knowledge of psychopathology, addiction, behavioral health, the ASAM criteria, and knowledge of life domain needs assessment.
