Job Description:
Clover Meadows Rehabilitation and Healthcare Center is looking
for a dedicated and compassionate Licensed Practical Nurse (LPN) to
join our team of healthcare professionals, committed to providing
exceptional care and rehabilitation services. As an LPN at Clover
Meadows, you will be a crucial member of our nursing team,

with our resident-focused philosophy. This role involves direct
patient care, treatment, and the management of health conditions,
as well as the facilitation of patient recovery to improve quality
of life. Provide direct nursing care in accordance with resident
care plans Collaborate with multidisciplinary team members to
enhance patient rehabilitation and recovery Monitor patient
progress and adjust care plans as needed Administer medications and
treatments accurately and safely Maintain records of patients’
health conditions and care Ensure compliance with all health and
safety regulations Communicate effectively with residents,
families, and team members Participate in training and educational
opportunities Strong clinical skills and knowledge Excellent
communication and interpersonal skills Proficiency in managing
multiple tasks Detail-oriented and efficient in documentation
Medical Insurance Dental Insurance Vision Insurance Current LPN
