  • Under the guidance of the Chief of Perioperative Services examine patients referred to the PAT. Work collaboratively with the surgeons and anesthesiologist within the scope of his/her practice to diagnose and treat patients, and where required, recognize and determine the need for additional health screening so patient can be medically optimized for surgery.
  • Order blood tests, EKG, urinalysis, radiology exams and any additional testing, evaluate and interpret patient data to determine patient readiness for surgical intervention.
  • Maintain appropriate documentation and collaborate with other members of the interdisciplinary team to determine, evaluate and achieve medical optimization.
  • Provide comprehensive exam and education as needed for patient as needed.
  • Initiate referrals and counsel patients and families accordingly as it relates to one's health status, goals of surgery.
  • Make necessary medication changes and provide education on medication management for patient prior to surgery.
  • Consult with clinical team to ensure all testing needed is conducted prior to surgery date.
  • Encourage practices that promote health, well-being, reduce anxiety, stress and disability.
  • Document all encounters, plans of care and patient related interventions in the EPIC electronic medical record
  • Perform all other duties assigned by the Chief of Perioperative Services, Chief of Nursing, Chief Medical Officer or designee.Minimum Qualifications
    • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
    • Current certificate as a Nurse Practitioner issued by the NYSED; and
