The Care Manager/Recovery Coach works in partnership with service
participants to facilitate individual mental health and
co-occurring substance use recovery. This includes community
integration and role development accomplished by engagement and
assertive outreach, crisis intervention and diversion, support
network building, recovery planning, and support in the exploration
and use of community resources. JOB DUTIES AND RESPONSIBILITIES:
Provide individualized case management services to adults and/or
children and families in places such as their home, work, school,
or other locations within the community.
Provide assistance with developing skills and resources for social
determinants of health. These can include housing, food,
educational/vocational services, medical care,
entitlements/benefits, and social support.
Build good working relationships with both internal and external
team members.
Participate in the creation of key community partnerships that
facilitate participant recovery.
Maintain a flexible schedule as needed to meet the needs of
participants.
PHYSICAL AND SENSORY REQUIREMENTS:
Must have ability to sit for 2 total hours per day, and up to 1
total consecutive hour. Must be able to stand for 8 total hours per
day and up to 2 total consecutive hours. Must be able to walk 6
total hours per day and up to 10 total consecutive minutes. Must
have the ability to frequently use fingers/hands for fingering and
handling. Must have the ability to use upper extremities for
frequent pushing of objects up to 10 pounds and occasional pushing
of objects up to 75 lbs. Must have the ability to transport
patients weighing up to 250 lbs. via wheelchair, stretcher and/or
bed. Must have the ability for frequent stooping and bending. Must
have the ability for frequent reaching above shoulder level. Must
have the ability for touching as it relates to feeling objects by
touching with the skin, particularly that of fingertips. Must have
the ability for hearing as it relates to normal conversation, high
frequency and low frequency hearing sounds. Must have the ability
for seeing as it relates to general vision, near vision, color
vision and peripheral vision.
EDUCATION:
Bachelors degree which includes or is supplemented by at least 12
credit hours in a related field
Or registered nurse
Or a high school diploma along with 12 semester credit hours in
sociology, social welfare, psychology, gerontology, or other
related social sciences and 2 years experience in direct contact
with mental health consumers. Internship and volunteer experience
will be considered.
Bachelors degree is preferred.
TRAINING AND EXPERIENCE:
Must understand the principles of mental illness, and behavioral
best practice and crisis management planning.
WORK SCHEDULE:
Monday through Friday days with flexibility for early evenings
hours.
OTHER:
This position requires one day per month of on-call coverage of
department crisis phone, with additional compensation. There is
opportunity to take additional shifts as desired.
