Patient Safety Manager Location Name Riverside Behavioral Health Department Quality Management Job ID 2021-25772 Category Leadership (Non-Nursing) Shift Days Overview The Patient Safety Manager/ Patient Advocate works under the supervision of the Director of Quality and is responsible to investigate resolve document and report Riverside Behavioral Health Center specific patient and visitor complaints. This role will develop implement and participate in customer service and patient relation initiatives. This role is the champion for patient and team member safety and responsible for the implementation of the safety program following evidenced based safety behaviors and practices focusing on people process and technology to improve safety related performance. Follows all Department of Behavioral Health Office of Human Rights guidelines. Responsible for the oversight and entries within the CHRIS Database and internal incident reporting system. Conducts facility rounds engages team members and creates an environment that is safe for all patients and residents. Oversees and coordinates performance improvement activities in an effort to enhance quality and reduce and/or prevent medical errors. Participates and leads projects related to Performance Improvement/outcome measures licensure/accreditation Risk Management and Medical Record documentation. Ensures timely PI data collection and entry for licensure and regulatory agencies such as: DNV CMS Core Measures and RHS/RBHC quality and scorecard indicators. This position will analyze data from multiple sources aggregate the data and present the data in a usable format. Participates in licensure and accreditation surveys to include data collection outcome results analysis action and follow-up implementation. This role supports a learning environment focused on team member and patient safety provides training for new hires and as needed for team members and medical staff on safety measures complaint and grievance processes service recovery and reporting requirements. The intent of this position is to serve in both a proactive and reactive manner in order to pre-identify issues and in a reactive manner to facilitate performance improvement and service recovery. Applies just culture principles to assure they are evident throughout the organization.

            Riverside Behavioral Health Center offers the most comprehensive range of behavioral health programs on the Virginia Peninsula. An established mental health care provider in Hampton Roads since 1983, Riverside Behavioral Health Center provides psychiatric and chemical dependency services to children, adolescents and adults on an outpatient and inpatient basis. We offer a variety of services such as Inpatient Adult Services, Inpatient Child and Adolescent Services, Adolescent Residential Services and Outpatient Services, to individuals who experience emotional, psychological and behavioral problems. Individual, family and group therapy, recreation therapy, art therapy, life skills groups, and medication management are key components of the programs we provide. Qualifications Education:
            • Bachelors Degree required in Counseling, Nursing, or healthcare related field
            • Masters degree can be accepted as well.
            • If no Bachelors Degree, must show proof of current enrollment in a related Bachelors Degree program with completion of degree no later than 2 years from date of hire. Experience:
              • 2 years of relevant experience in hospital healthcare quality, safety, and/or risk management required.
              • Experience in project development, project management, and strong analytical skills preferred.
              • Must have knowledge of patient safety and advocacy requirements.

                Certifications:
                • CPPS required within 6 months of hire,
                • CPHRM required within 6 months of hire
                • CPR and First Aid required within 30 days and maintained thereafter
                • RN license preferred
                • Lean and or Six Sigma Certification and experience preferred.
