Supportive services include engagement, assertive outreach, crisis intervention and diversion, strengths-based assessment and recovery planning, support network building, and mentoring/coaching/ instruction in the exploration and use of community resources with participants meeting the service criteria described in Pennsylvania Code Title 55, Chapter 5221 and in accordance with Penn Foundation policy.JOB DUTIES AND RESPONSIBILITIES:

  • Provide individualized recovery coaching services at community locations such as home, work, street, or school as needed and clinically appropriate.
  • Provide services based on a holistic approach to wellness and psychiatric rehabilitation to include recovery-oriented engagement and cooperative partnership development; community-based outreach; assessment of strengths and needs; stage of change-matched recovery education and integrated wellness planning; and provision of vision-driven hope, encouragement, and support in experiencing community resource - - exposure/linkage/acquisition and reciprocal integration opportunities. -
  • Assist participants in choosing, getting, and keeping self-determined life goals and roles including providing assistance with developing the skills and resources defined in the individualized recovery/integrated wellness plan that will meet their basic needs (housing, food, education/vocational services, medical care, financial entitlements and social support) and maximize their potential for successful and satisfying community living.
  • Assist participants in identifying triggers, risk factors and other potential challenges to their recovery plan and create with the participants strategies to address these issues.
  • Educate participants regarding tools and opportunities for self-advocacy.
  • Function as the external team leader and ensure coordination and collaboration with all involved treatment and rehabilitation staff and applicable community members in each participant's support team.
  • Identify the natural supports that already exist within the daily experience of participants and determine with the participants how to elevate/enhance this support including participation in mutual aid/peer support.
  • Participate with the Team Leader, fellow staff, and participants in creating key community partnerships that facilitate participant recovery and "give-back" opportunities.
  • Provide required number of available hours in billable, direct service activity.
  • Maintain a flexible work schedule, including evening and weekend hours as needed to meet the needs of referrals, participants, and the department.
  • Participate in rotating on-call services as required.
  • Utilize de-escalation and crisis diversion skills and strategies to support participants who are in crisis in addition to crisis planning prior to a crisis takes place. If hospitalization cannot be avoided, support participant through hospitalization and act as liaison with hospital and treatment team to ensure safe and smooth transition back to the community.
  • Complete required agency, county, state, and funding source documentation of Recovery Coaching/Blended Case Management services in a thorough and timely fashion. -
  • Attend departmental supervisory, clinical, and other meetings as required.
  • Participate in agency integrated team(s) as developed/implemented/required.
  • Possess and maintain a valid driver's license/safe driving record.
  • Perform related work as requested/assigned.PHYSICAL AND SENSORY REQUIREMENTS: Must have ability to sit for 2 total hours per day, and up to 1 total consecutive hour. Must be able to stand for 8 total hours per day and up to 2 total consecutive hours. Must be able to walk 6 total hours per day and up to 10 total consecutive minutes. Must have the ability to frequently use fingers/hands for fingering and handling. Must have the ability to use upper extremities for frequent pushing of objects up to 10 pounds and occasional pushing of objects up to 75 lbs. Must have the ability to transport patients weighing up to 250 lbs. via wheelchair, stretcher and/or bed. Must have the ability for frequent stooping and bending. Must have the ability for frequent reaching above shoulder level. Must have the ability for touching as it relates to feeling objects by touching with the skin, particularly that of fingertips. Must have the ability for hearing as it relates to normal conversation, high frequency and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision. EDUCATION:A bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, nursing, other related social sciences, criminal justice, theology, nursing, counseling, or education; OR a registered nurse. - TRAINING AND EXPERIENCE:Able to demonstrate understanding of serious mental illness and integrated health issues including but not limited to co-occurring substance use, trauma, and medical concerns. - WORK SCHEDULE:Monday through Friday days with flexibility for early evenings hours. Position requires one day of on-call coverage of department crisis phone per month. On-call shifts are compensated beyond base salary and there is opportunity to take additional on-call shifts as desired.
